Grease trap service record and manifest template

By Stew Israil, GreaseTrapRules · Updated Oct 19, 2026 · Every rule below links to its official source. Rules change; verify with your city.

Most cities that regulate grease traps want proof of every pump-out: a hauler's manifest, a service record or a pumping log kept on site. The exact fields differ. Below are the record rules of the 85 cities in our data that publish one, quoted from the official source, and a checklist of standard fields for places that don't spell it out.

Standard fields for a grease trap service record

These are the fields our tool includes. Where a city rule names a field, it's quoted in the table below; the rest are standard industry fields, not a legal requirement everywhere.

Florida: the statewide manifest rule

Florida law requires a service manifest for every grease trap or interceptor pump-out, showing the trap's condition, that it was cleaned, the amount removed and the disposal facility, signed by the hauler and the restaurant. The restaurant keeps a copy on site for 1 year (Florida Statutes § 403.0741 (grease waste removal and disposal)). Cities like Tampa, FL and Jacksonville, FL add their own rules.

FAQ

Is there an official grease trap manifest form?

How long do I keep grease trap records?

It depends on the city. The rules above range from 1 year (Florida statute) to 5 years in some cities. Check your city's rule.

Is the service record tool free?
